How does Help at Home by Montefiore work?

Call or email Help at Home by Montefiore any time. Our Coordinator will speak to you about your support needs, then pay a complimentary visit to your home to discuss and develop a plan matching your needs with our services.

We currently serves clients in the Eastern Suburbs, Northern Beaches, Upper & Lower North Shore, Hunters Hill, Ryde, North West & Hills District, Mosman & surrounds and the Inner West.

Help at Home supports both Consumer Directed Care (CDC) packages and Fee for Service clients.

Consumer Directed Care (CDC) packages

To apply for a CDC, you first need to be assessed and approved for a package by an Aged Care Assessment Team (ACAT). You can self-refer, get a referral via your GP, or our Help at Home team is happy to assist and refer you. Once you've had your package approved, a care coordinator will help tailor the best care options for you. For more information on ACAT and how to apply for a package, visit: www.myagedcare.gov.au

Fee for service care

Anyone can access Fee for Service care through Help at Home by calling 1800 251 537. Our staff will provide a verbal quote and payment is by credit card. If your service requirements are more extensive, we will put together an Agreement to formalise your care arrangements.
