How does Help at Home by Montefiore work?

You can access our wide range of services via government-subsidised Consumer Directed Care (CDC) packages, through the National Disability Insurance Scheme (NDIS) or as private Fee for Service care. Help at Home is happy to support you to apply for these subsidies, or make a private arrangement.

Help at Home currently works with clients in the Eastern Suburbs, Northern Beaches, Upper & Lower North Shore, Hunters Hill, Ryde, North West & Hills District, Mosman & surrounds and the Inner West.

You can contact us any time and speak to an expert Care Coordinator about your support needs. We will then pay a complimentary visit to your home to discuss and develop a plan matching your needs with our services.

Consumer Directed Care (CDC) packages

Older members of the community may qualify for a Consumer Directed Care (CDC) package, offering government subsidies for care and services to help you live independently. To apply, you can self refer for an evaluation by an Aged Care Assessment Team (ACAT) at myagedcare.gov.au, get a referral from your GP, or our Help at Home team is happy to assist you.

National Disability Insurance Scheme (NDIS) participants

People living in the community with a disability may be eligible for government subsidised NDIS services that support you to live in the community. Help at Home can assist you to apply for funding, then help to bring your NDIS plan to life by offering support coordination and plan management.

Fee for service care

If you don’t qualify for CDC or NDIS subsidies or are waiting for approval, Help at Home also offers private, fee-for-service care. Call our hotline for a verbal quote or to arrange a consultation.
