How does Help at Home by Montefiore work?
Call or email Help at Home by Montefiore any time. Our Coordinator will speak to you about your support needs, then pay a complimentary visit to your home to discuss and develop a plan matching your needs with our services.
We currently serves clients in the Eastern Suburbs, Northern Beaches, Upper & Lower North Shore, Hunters Hill, Ryde, North West & Hills District, Mosman & surrounds and the Inner West.
Help at Home supports both Consumer Directed Care (CDC) packages and Fee for Service clients.
Consumer Directed Care (CDC) packages
To apply for a CDC, you first need to be assessed and approved for a package by an Aged Care Assessment Team (ACAT). You can self-refer, get a referral via your GP, or our Help at Home team is happy to assist and refer you. Once you’ve had your package approved, a care coordinator will help tailor the best care options for you.

Fee for service care
Anyone can access Fee for Service care through Help at Home by calling 1800 251 537. Our staff will provide a verbal quote and payment is by credit card. If your service requirements are more extensive, we will put together an Agreement to formalise your care arrangements.
